Aesthetics Lasers

Application Details:

Aesthetics lasers are typically utilized for surface treatments, but can also penetrate the tissue at different depths depending mostly on wavelength. Different wavelengths have different absorption and transmission rates for various tissue types, and are therefore utilized for different light based treatments or procedures. Lasers can treat various skin conditions, minimize or remove spots on the skin, soften and minimize scar tissue (including acne scars), tighten skin and smooth wrinkles, minimize pigment issues, stop hair growth, promote wound healing and much more. Here are some more details on a few of these specific applications:

Laser Tattoo Removal:

Similar to laser hair removal which we discussed in a previous blog, laser tattoo removal utilizes a process known as selective photothermolysis to target the embedded ink in the epidermis and dermis.  Photothermolysis is the use of laser microsurgery to selectively target tissue utilizing specific wavelengths of light to heat and destroy the tissue without affecting its surroundings.  In laser tattoo removal this is accomplished by using a focused q-switched laser with a fluence of approximately 10 J/cm², to heat the ink molecules locally.  Since the q-switched laser’s pulse duration (100 ps to 10 ns) is shorter than the thermal relaxation time of the ink molecules it prevents heat diffusion from taking place.  In addition to minimizing damage to the surrounding tissue, this rapid localized heating results in a large thermal differential, resulting in a shock wave which breaks apart the ink molecules.  Once reduced to smaller constituents, the molecules can now be absorbed by the white blood cells and transported to the liver for processing out of the body through urination. Laser Hair Removal:

To understand how laser hair removal works, understanding the growth cycle of hair follicles is essential. As shown in the image to the right, there are three different stages in the hair growth cycle: anagen (growth), catagen (atrophy), and telogen (dormancy). Of these three stages, laser hair removal is only effective during the anagen stage of growth. At this stage, the laser light can transmit through the epidermis/dermis and be directly absorbed by the hair, burning out and killing the dermal papilla, rendering it permanently inactive. To understand which type of laser is ideal for this application, we must look at the absorption properties shown in the figure to the right. From this image, we can see that in the 700 nm to 1000 nm region, there is a dip in both the hemoglobin and water absorption, which correlates not only to Alexandrite but also GaAs. When laser hair removal started, it was initially far more efficient to generate the high laser flounces needed (20 J/cm2 or greater) via Alexandrite laser technology, which can easily produce 60 J pulses at 755 nm. With the recent developments in diode laser technology, companies have been producing highly efficient and reliable 808nm and 940nm diode stacks with high output powers into the kilowatt range. Laser Acne Treatment:

While there have been studies performed showing some efficacy when utilizing blue lasers and LED light sources to specify target and kill the p. acnes bacteria, it only provides up to 40% decrease in acne lesions.  The procedure requires continual treatment (up to 14 hours per month) because bacteria’s food source (the oil) is left unaffected,  and it has been shown that patients with cystic acne experience a worsening of symptoms when exposed to blue light.  Utilizing near-infrared light instead focuses on the photothermal heating of the mid-dermal layer of the skin where the sebaceous glands are located, effectively burning them out.   The localized heating is due to the light’s depth penetration. Wavelengths between 1400 nm and 1500 nm are ideal for targeted absorption in the middle of the dermis, without damaging either the skin’s surface or the subcutaneous fat. In their initial work, a group led by Dr. Friedman at the University of Texas Houston School of Medicine, and MD Anderson Cancer Center utilized non-over lapping 6mm diameter 1450 nm laser pulses with fluences between 10 J/cm² and 14 J/cm² for the treatment.  This treatment performed once every 4 to 6 weeks, show an average decrease in acne lesions counts of 37% after one treatment, 58% after two procedures, and 83% after a third.   Read the full article here.
